The Dental Consent Form is required for any dental prophylaxis or tooth removal surgery. This form gives our veterinarians permission to anesthitize your pet for the dental procedure that has been requested and gives some options for your pet. Please read the form completely before signing and bring the form with you at the time of your pet's procedure. Please call us with any questions.
