Treatment Release
This form is used whenever you leave your pet in our care for any reason other than anesthesia or surgery. It has prompting questions regarding their recent medical history and a release enabling the doctors at Pyramid Veterinary Hospital to examine and treat your pet without your immediate presence.
Please note: If your pet will be sedated or anesthesia used, the Anesthesia Consent Form will be needed, not the Treatment Release Form.
